Job Details: Schedule: Monday–Friday | 8-Hour Shifts | 40 Hours/Week Location: Lynchburg, VA Job Type: Contract Contract Length: 8 Weeks Guaranteed Hours: 40 Hours/Week Pay Range: Based on experience, expertise, credentialing, and education. Duties: Provide skilled nursing care to patients in their homes under RN supervision. Assess patient needs and report changes to the RN Case Manager. Assist with care planning, patient education, admissions, discharges, and transfers. Provide hospice/end-of-life care when applicable. Maintain accurate clinical documentation and coordinate with the interdisciplinary team. Requirements: Education: Graduate of an accredited LPN/LVN program. License(s): Active LPN/LVN license in the practicing state. Certification(s): Current CPR certification. Years of experience: 2+ years of LPN/LVN clinical experience; home health/hospice preferred. Skills/Specialties/Technology: Home Health/Hospice, patient assessment, care coordination, Homecare Homebase (HCHB) , electronic documentation. Valid driver's license, reliable transportation, and current auto insurance required.
