The Role We’re hiring a Nurse Case Manager to be the trusted clinical guide for our families — from hospital discharge through everyday life. You’ll coordinate care, teach and support family caregivers, and keep the team aligned (physicians, payers, supplies, nursing staff) so kids get consistent, high-quality home care without gaps. Key Responsibilities Own the Care Journey Serve as the primary clinical point of contact for assigned pediatric patients/families. Convert complex discharges into clear, at-home care plans; coordinate orders, authorizations, and supplies. Prepare complete, timely prior auths to reduce denials and coverage gaps; track expirations/renewals. Review documentation for completeness and share simple quality metrics (e.g., missed visits, readmits). Support & Teach Caregivers Coach family caregivers via training modules and real-time check-ins. Teach meds, pumps, vents, g-tubes, and safety in plain language ; celebrate progress and troubleshoot calmly. Lead the Care Team Match families with the right nurses; fill gaps quickly to avoid missed care. Conduct routine home visits for quality/safety; provide feedback to field staff. Build Better Processes Use our tools to streamline documentation/scheduling. Flag workflow friction, propose fixes, and help roll out improvements across regions. Grow with Us Partner with the Director of Nursing on initiatives and special projects. Mentor new nurses; help scale a model that’s expanding statewide. Required Qualifications License/Certs: Active Florida RN License; BLS required ; PALS within 3 months of hire. Experience: 4+ years total nursing , including 2+ years pediatrics (PICU/NICU/peds clinic or home health). 2+ years home health case management (pediatric preferred). Skills: Authorization workflows (Medicaid), medical necessity, documentation quality, caregiver teaching. Tools: EMR + basic spreadsheets/messaging platforms. Other: Valid driver’s license and reliable transportation for local visits. Language: Bilingual English/Spanish required.
