Description: RESPONSIBLE FOR: Responsible for providing indirect patient care by ensuring that the list of home medications is completed per Policy and Procedure at the time of admission and assisting physicians and midlevel providers with transfer/discharge medication reconciliation, when requested. Qualifications: MINIMUM EDUCATION REQUIRED: Graduate of a nursing program MINIMUM EXPERIENCE REQUIRED: Two (2) years experience in an Inpatient Nursing Unit MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW: Current License in the State of Georgia as a Registered Nurse or NLC/eNLC Multistate License.
