HSS ABBREVIATED JOB DESCRIPTION Position: Homecare Services Supervisor Mission: To establish a safe, healthy living environment for consumers through exceptional caregiving and excellent peer leadership to a team of caregivers with the goal of ensuring that every senior served by the agency has access to our services and can live with dignity and respect in the community, rather than an institution. Reporting to: Region Director Responsibilities of the Home Services Supervisor Position: Direct Caregiving -- Homecare Services Supervisor (HSS) is first and foremost a caregiver responsible for the needs of the consumers. These needs include personal care, activities, socialization, cleaning, meals, laundry, and assistance with medication. Responsibilities of a typical day may include, but not be limited to: Receiving report from the previous shift Conducting money & narcotics count, as indicated, with previous & next shift staff Checking calendar for appointments & ensuring clients are prepared for them Waking clients and assisting them in getting bathed, groomed, & dressed Preparing, serving and cleaning up after each meal Helping and encouraging clients with activities and social interactions Helping clients with toileting and making sure that they are always clean and comfortable Assisting clients with prescribed medications Preparing for second shift 2. Team Leadership -- HSS is responsible for the staff and house schedule ensuring that all shifts are staffed, call offs are managed efficiently, and appointments and visits are coordinated. Responsibilities include, but are not limited to: Instilling a culture with caregivers and clients that this is the client’s home and that all caregivers “work where the clients live; the clients don’t live where the caregivers work.” Leading by example and by words that treat clients with the highest dignity and respect, even for clients who are challenging. Assisting director in completing weekly/monthly schedule when shifts are unassigned, ensuring that all shifts are covered consistently and without the need for overtime. Managing call offs as assigned by the director -- Call off management is included in the 1.5 hours per week of overtime pay for administrative duties. Should call offs exceed 1.5 hours per week, call offs are then managed by director. Discussing options with caregivers to prevent call offs whenever possible Contacting regular caregivers (caregivers who regularly and previously had worked in the HSS’s house) and requesting coverage, whether switching with other caregivers or accepting additional hours Reporting call offs to director in a timely manner Discussing call offs with caregivers at next scheduled shift to ascertain the reason for the call off, counseling caregiver about potential consequences for call offs and encouraging continued teamwork and commitment to the house schedule e. Regularly engaging staff in discussions about their tasks, activities on their shifts, and immediately notifying director when staff indicate that their availability may change or they are seeking other employment. 3. Training, Coaching, and Mentoring -- HSS is responsible for the quality of life that each client experiences in their home. As a result, the team leader is expected to teach and communicate the standards of care and experience that is expected. Responsibilities include, but are not limited to: Creating an environment tailored to the individual needs and preferences of each client in the home Understanding and anticipating the needs and preferences of each client Orienting and training newly hired caregivers, providing feedback to the director to ensure that all staff are thoroughly trained and competent to address client care needs Educating caregivers of emergency procedures and agency policies Developing knowledge of the caregivers in the home, recognizing the strengths and weaknesses of each caregiver and utilizing these strengths and weaknesses appropriately and to the benefit of the team Coaching and training each caregiver by giving constructive advice, giving praise when appropriate and correcting actions when necessary; expressing appreciation for their contribution and making personal connections. Holding the team accountable to established standards for safety and well being of the clients Always being available to the team and the clients Always providing training in order to cultivate new team leaders 4. Operation and Organization -- HSS is responsible for the overall orderly functioning of their home, as well as the perception of the agency. Any visitor should be able to see that the home is warm and inviting, well run and orderly. Responsibilities include, but are not limited to: Running the home using high levels of organization Ensuring that cleanliness and presentation of the house is consistently excellent Maintaining accurate documentation Being familiar with the environment and location of all supplies Keeping the food and supply inventory up to date Overseeing and reporting maintenance issues with accuracy and detail Ensuring clients are never without critical supplies, including but not limited to, ordering Attends from Service Coordination entities. b. Activity planning and coordinating -- A primary role of the HSS is to facilitate community integration to the greatest extent possible, as seniors need to remain connected to their community to sustain a sense of belonging and security. HSS is responsible for two types of activities for clients: i. Rhythm of Life Activities -- These are normal, daily tasks that can include things such as sharing family recipes, assisting with folding laundry or drying dishes, reading and discussing a newspaper, sharing family stories, etc. These are the types of activities that turn a house into a home and encourage the clients and staff to become a “cooperative family.” ii. Planned Activities -- These are activities that are scheduled by gathering input from clients and caregivers to develop enjoyable activities for each day of the week. HSS is responsible for ensuring that these activities are posted, carried out, documented, and communicated to family members and management. c. Meal planning and ordering i. Gathering input from clients and caregivers to develop nutritious and enjoyable meal plans. Meal plans should be individualized for the clients and should be considered one of the most important aspects of the day. Meal plans should also take into consideration the cooking abilities of their team members on each shift (i.e., don’t plan meals that a team member will have difficulty preparing) ii. Following dietary restrictions iii. Creating and posting weekly menu iv. Submitting menu and grocery list to director for approval v. Ensuring clients always have access to food when and where they choose vi. Ensuring clients always know they are free to choose alternatives to the planned meal d. Coordinating appointments i. Ensuring that appointments are scheduled with the correct date, time, location and provider. This information is communicated with the client, caregivers, family, and director. ii. Ensuring that transportation is secured well in advance iii. Ensuring that any necessary follow up is completed and instructions are carried out accurately and in a timely manner e. Assistance with medication i. Assisting clients in keeping medications stored properly and safely ii. Ensuring that Specialized Care Services Logs are kept up to date and the Log is completed accurately by caregivers and clients iii. Ensuring that all caregivers are properly trained and familiar with medication routines for clients that need assistance, either with reminders or with physical assistance with self-administration of their medications 5. Oversight of the House Routine -- HSS is responsible for ensuring that a caregiver is in the home 24/7, that all house tasks are completed according to schedule, and duties are delegated fairly among caregivers. Responsibilities include, but are not limited to: LEAF responsibilities: Ensuring that daily LEAF documentation is completed accurately during each shift Reviewing daily LEAF documentation for clients each day Ensuring that services are provided according to each client’s care plan Contacting Director with any questions about the house schedule or if there are any technical issues with the tablet that need to be addressed in a timely manner Completing paper LEAF documents during times when the tablet is not available or functioning properly and ensuring other caregivers are doing the same. b. Routine of Care -- HSS documents an in-depth explanation of a typical day in the client’s life so that others can quickly become familiar with each client’s routine. This information is kept in paper form or can be entered into LEAF as part of the client notes section. c. Household tasks/chores -- HSS delegates tasks and ensures that all are completed in a timely manner. HSS takes into account the strengths and weaknesses of their caregivers in these areas. HSS also takes into account house routines for the most appropriate times to complete these tasks. d. Reporting -- HSS is responsible for ensuring that the house is safe, clean and truly the client’s home. Duties include the following: i. Creating list and calling in maintenance requests, prioritizing those that could jeopardize the safety of the clients and staff. Building and grounds requests are reported to Senior Shared Housing (SSH) call center. ii. Ensuring that all equipment and appliances are in good working order Position Fit – Position Competencies: Mentorship -- Demonstrates ability to coach caregivers to improve performance and quality of care for consumers; models competence, responsibility, compassion and professionalism. Follow-through on commitments -- Sees all tasks through to completion and by deadline; lives up to verbal and written agreements, regardless of personal cost. Professional demeanor -- Maintains calm, stable performance when under heavy pressure or stress; shows respect and consideration to others in all situations; represents the agency in a mature, ethical, and responsible manner at all times.
