Position Title : Care Manager Hours : M-F 8:00am – 5pm Pay Rate : $32/hr Location : Field based Sarasota, FL Requirements : · 2+ years Field case management experience · Experience with High volume caseload (50+ cases) · Experience with electronic medical records · Strong computer skills – MS office · Bachelor’s Degree · Valid Driver’s License · Bilingual in Spanish preferred Job Description: Responsibilities: · Develop and maintain individualized long-term care plans, identifying appropriate providers, specialists, community resources, and support services to address member needs. · Coordinate care among members, families/caregivers, healthcare providers, and community partners to ensure access to necessary long-term care services and resources. · Monitor member progress, care plans, and outcomes, making recommendations and adjustments to support optimal health, independence, and quality of life. · Educate members and caregivers on available services, healthcare benefits, provider recommendations, community resources, and care management processes.
