Requirements
Current Nursing License in the practicing state
Valid Driver's License
Current CPR Certification
Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice
Proof of current Hepatitis consent/declination and TB vaccination record
Ability to use electronic medical charting and time-keeping software (Homecare Homebase)
Responsibilities
Assist in providing nursing services and identifying the patient's physical, psycho-social, and environmental needs under the direction of a Registered Nurse (RN).
Participate in planning, coordinating, and implementing care in conjunction with the RN.
Provide effective communication to patients/families and other healthcare professionals.
Assist patients/families in learning appropriate self-care techniques and assess responses to care.
Utilize infection control measures to protect staff and patients.
Assist with the admission, discharge, and transfer of patients.
