Description
The Home Health Physical Therapy Case Manager evaluates, develops a plan of care and treats patients to maintain or restore patients to their maximum functional performance level and completes discharge planning. Provides supervision for Licensed Physical Therapist Assistants and Certified Nursing Assistants.

Implements action plans to achieve or exceed service excellence benchmarksEvaluates and develops plan of care according to American Physical Therapy Association (APTA) guidelinesAdministers physical therapy treatments appropriate to diagnosis and to APTA guidelinesAppropriately documents and synchronizes patient care according to federal, state, and local regulationsMeets productivity standardsUses resources (e.g., time, people, supplies, etc.) cost effectivelySupervises Licensed Physical Therapy Assistants, Certified Nursing Assistants and studentsProtects patients, employees, and visitors by promoting infection-control and safety policies and protocolsPlans, coordinates, manages, and documents the care provided to assigned patientsEnsures appropriate patient and/or caregiver education with utilization of applicable resourcesLeads, communicates and collaborates with the interdisciplinary team to plan patient careAdheres to National Patient Safety Goals (NPSG)Participates in performance improvement initiatives affecting patient outcomesHandles conflict resolution in an appropriate, timely, and consistent mannerIdentifies personal contributions to department cascading goalsPractices within physical therapy licensure guidelinesProvides one in-service annually to staffEngages in own professional development as evidenced by self assessment and attainment of degrees, certifications, and continuing educationContributes to providing a supportive environment that is conducive to learning for all students from all academic programsOther duties as assigned
Qualifications
Required Education: Bachelors Degree in Physical TherapyPreferred Education: Masters Degree in Physical Therapy, or Doctoral Degree in Physical TherapyRequired Experience: For Bachelors Degree, five (5) years of experience as a Physical TherapistPreferred Experience: Skilled Nursing Facility experienceRequired Certifications and Licensures: Licensed by the Virginia Board of Physical Therapy Salary Range:  $83,588.00 - $125,382.00
