Job Overview We are seeking a Program Manager as a visionary and entrepreneurial leader responsible for designing, launching, and scaling a Food Is Medicine initiative within our Home Delivered Meal service model. The Manager will provide day-to-day oversight and management of the program including coordination, supervision, and administrative oversight of the client services program team. This role will lead all aspects of pilot program development, clinical outcomes monitoring, training, delivery, partnership cultivation, and strategic growth initiatives. The position requires a dynamic leader who combines operational excellence, case management expertise, business development acumen, and a passion for improving the quality of service. Strategic Leadership Lead the development and launch of a Food Is Medine program aligned with organizational mission and long-term growth objectives. Function as an expert clinical practitioner, case management subject-matter expert, resource, advisor and leader for the members of the client services program team. Establish and execute strategic goals, operational plans, and performance metrics for all programs. Build scalable systems and infrastructure to support expansion. Collaboratively develop annual budgets, forecasts, and sustainability plans for operations. Represent the organization at conferences, industry meetings, and community events. Quality Assurance Supervise and monitor client services team to ensure effective care coordination and case management practices are consistent with organizational policies, and applicable regulations and guidelines. Establish evaluation systems that monitor the quality of interactions, scheduling, delivery, documentation, and assessments of program participants to align with organizational goals. Partnership Development & Community Engagement Build strategic partnerships with healthcare providers, hospitals, rehabilitation centers, contracted agencies, and community organizations. Develop referral pipelines for program participants. Identify opportunities for sponsored pilot programs, grants, and collaborative initiatives. Promote the organization through outreach, presentations, networking, and business development activities. Business Development & Growth Strategy Identify revenue-generating opportunities including Medicaid, Medicare Advantage and private healthcare systems. Support marketing and recruitment efforts to increase participation. Collaborate with leadership to position the organization as a recognized leader in Food Is Medicine initiatives and food security for seniors. Qualifications & Competencies: Bachelor’s degree in Social Work or Human Services; Master’s degree in Healthcare Administration, Education, Gerontology, Nursing, Social Work, Public Health, Business Administration, or related field preferred. Minimum of five years’ experience in case management, preferably in elder care or social services setting. Entrepreneurial mindset with ability to innovate and scale programs strategically. Ability to build programs from concept through implementation and growth. Exceptional leadership, organizational, and project management skills. Strong business development and relationship-building capabilities. Excellent written, verbal, and presentation communication skills. Ability to analyze data and use outcomes to drive continuous improvement. Understanding and focus on continual learning of compliance regulations in the industry. Proven passion to uphold and support the mission of Meals on Wheels. Ability to work well both independently and in conjunction with others. Coachable and open to ongoing professional development. Strong partner and team player. Professional yet approachable in dress and demeanor. Proficient in Microsoft Office (Word, Excel, Powerpoint and Outlook) Must be authorized to work in the United States.
