Position Overview The Outreach Case Manager for the Pottstown program serves as a critical link for individuals experiencing homelessness in Montgomery County, PA. This role focuses on field-based outreach to engage vulnerable populations, build trust, and facilitate connections to housing, health care, and essential resources. The position requires a Housing-First and Trauma Informed Care (TIC) approach, a commitment to harm reduction, and the ability to work collaboratively within the PA-504 Continuum of Care (CoC). Key Responsibilities Outreach and Engagement Conduct field-based outreach to encampments and locations where individuals experiencing homelessness reside. Provide real-time, responsive crisis intervention and support for vulnerable populations, including pregnant women, children, and those with medical conditions. Utilize motivational interviewing and harm reduction techniques to build rapport and address immediate basic needs (food, health, transportation). Maintain an ongoing caseload of homeless clients/households, ensuring consistent engagement and case management. Case Management and Resource Coordination Provide Coordinated Entry intake, assessment, and referral via HMIS to connect clients to housing resources. Develop individualized goal plans and monitor progress to ensure successful transitions from unsheltered to housed situations. Collaborate with housing resources and other partner agencies to navigate complex systems and advocate client needs. Facilitate linkages to public benefits, clinical care, legal resources, and behavioral health services. Provide transportation assistance to ensure clients can access appointments and essential services. Data, Compliance, and Reporting Maintain accurate documentation in HMIS and ensure all client files meet program standards (intakes, signed releases, service plans). Contribute to quarterly performance reporting, including data on demographics, housing outcomes, and service outputs. Participate in community-oriented encampment resolution, welfare checks, and coordinated outreach team activities. Qualifications Education: Associate degree or a combination of training and education in social work or a related field is preferred. Experience: Minimum one (1) to three (3) years of social services or case management experience working with homeless populations (or personal experience with homelessness). Knowledge/Skills: * Proficiency with standard business PC applications, e-mail, and the Internet. Strong interpersonal, oral communication, and conflict-resolution skills. Ability to work with diverse populations and maintain sound judgment in crisis situations. Licensing: Valid Pennsylvania Driver’s License required; ability to perform routine travel for job-related duties. This position follows a Friday through Monday work schedule, consisting of four 10-hour shifts each week. The exact schedule will be discussed with the hiring manager; however, example shift times include 8:00 a.m. to 6:00 p.m. or 9:00 a.m. to 7:00 p.m. Physical Demands and Work Environment The employee must be able to perform field-based work, including walking, standing, and lifting/moving up to 25 pounds. Work will involve visiting outdoor encampments and various community locations; reasonable accommodation will be provided to enable individuals with disabilities to perform essential functions.
