Duties Assist clients with activities of daily living (ADLs), including personal hygiene, grooming, and mobility support. Monitor patients' health status by observing vital signs, medication adherence, and overall well-being. Provide companionship and emotional support to promote mental health and social engagement. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary. Support clients with mobility using appropriate patient transfer methods such as Hoyer lifts or manual handling techniques. Maintain a clean and safe environment through regular cleaning, sanitation, and organization of living spaces. Administer medications following care plans while adhering to HIPAA regulations to ensure confidentiality and privacy. Assist with transportation for appointments or community activities, ensuring safety during travel. Document care activities accurately in compliance with healthcare standards and communicate effectively with healthcare teams. Support clients with specialized needs such as dementia, Alzheimer's disease, autism spectrum disorder, or developmental disabilities through tailored approaches. Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable. Knowledge of patient observation techniques, vital signs monitoring, medication administration, and first aid/CPR certification. Familiarity with care for individuals with dementia, Alzheimer's disease, neurodevelopmental disorders, or intellectual disabilities. Ability to perform heavy lifting and manual handling safely while adhering to proper patient transfer methods. Strong communication skills for effective client interaction and collaboration with healthcare teams. Experience working within HIPAA guidelines to maintain client confidentiality at all times. Demonstrated ability to provide compassionate patient personal care assistance and support independence. Valid certifications in first aid and CPR; additional training in hospice care or social work is advantageous. Flexibility to work various shifts including evenings, weekends, or overnight as needed.
