Nursing Care Coordinator – Berks Community Health Center Berks Community Health Center is seeking a Nursing Care Coordinator to lead a clinical support team and help deliver high-quality, patient-centered care in a fast-paced Federally Qualified Health Center (FQHC) environment. This role works closely with providers, nurses, medical assistants, and patient services staff to coordinate care, improve patient outcomes, and support continuity of care across the lifespan.
