About the Opportunity
Our client, a large nonprofit human services organization, is seeking a compassionate and experienced Registered Nurse to provide clinical oversight and care coordination for adults with intellectual and developmental disabilities living in community residential homes.
This is a unique opportunity for an RN who enjoys building relationships, working independently, and partnering with interdisciplinary teams to improve the health, safety, and quality of life of medically complex individuals. The position is primarily field-based with a combination of remote work, travel between residential homes, and occasional office meetings.
Rather than providing bedside nursing, this role focuses on care coordination, clinical oversight, staff education, and proactive management of individuals with complex medical needs.
Position Summary
The Registered Nurse serves as the clinical lead for a designated caseload of individuals, with a primary focus on those who are medically fragile or at higher risk due to chronic conditions, dysphagia, falls, or other complex healthcare needs.
Working closely with physicians, nurse practitioners, therapists, dietitians, QIDPs, residential leadership, and Direct Support Professionals, the RN ensures each individual receives appropriate medical care, active treatment, and person-centered support while maintaining compliance with state and federal regulations.
Responsibilities
• Coordinate healthcare services for individuals residing in community residential homes.
• Monitor the health status of individuals with chronic medical conditions and complex healthcare needs.
• Conduct nursing assessments and develop recommendations to support overall health and wellness.
• Serve as an active member of each individual's Interdisciplinary Team (IDT), collaborating on Individual Support Plans and ongoing healthcare needs.
• Coordinate physician appointments, specialist referrals, hospital admissions, and discharge follow-up.
• Ensure physician orders, medication changes, and treatment plans are accurately communicated and implemented.
• Provide clinical oversight for individuals requiring specialized care, including those at increased risk for falls, aspiration, dysphagia, or other complex medical conditions.
• Administer medications, injections, and nursing treatments as required.
• Train and educate Direct Support Professionals, Residential Assistants, and LPNs on:
• Medication administration
• Dysphagia and texture-modified diets
• Positioning and transfer techniques
• Infection prevention
• Fall prevention
• Medical protocols and documentation
• Monitor medication administration practices and ensure compliance with regulatory requirements.
• Supervise and mentor LPNs and provide clinical guidance to residential staff.
• Participate in quality assurance initiatives, incident reviews, infection control activities, and regulatory surveys.
• Maintain accurate nursing documentation and ensure compliance with applicable state and federal regulations.
• Serve as an advocate for the health, safety, dignity, and rights of the individuals supported.
Qualifications
• Active Pennsylvania Registered Nurse (RN) license.
• Minimum of two years of nursing experience.
• Experience supporting individuals with intellectual and developmental disabilities, behavioral health, medically fragile populations, long-term care, home health, community health, rehabilitation, or similar settings preferred.
• Strong understanding of chronic disease management, medication administration, and nursing assessments.
• Experience with dysphagia management, fall prevention, enteral feeding, or medically complex individuals is highly desirable.
• Excellent communication, documentation, and organizational skills.
• Ability to work independently while collaborating with interdisciplinary teams.
• Valid driver's license and willingness to travel between residential locations.
