Our client, a growing nonprofit organization supporting individuals with intellectual and developmental disabilities (IDD), is seeking compassionate Licensed Practical Nurses (LPNs) to join their clinical team.
This community-based position provides nursing oversight across several residential homes, partnering closely with residential staff, physicians, families, and interdisciplinary teams to ensure individuals receive high-quality, person-centered care.
What You'll Do
• Provide nursing oversight and clinical support for individuals living in community residential homes.
• Complete health assessments and monitor ongoing medical needs.
• Administer and oversee medications and treatments in accordance with physician orders.
• Coordinate medical appointments, specialist visits, and follow-up care.
• Communicate with physicians, pharmacies, hospitals, and other healthcare providers.
• Develop, implement, and update individualized healthcare plans.
• Train and educate Direct Support Professionals on medication administration, health-related protocols, and best practices.
• Monitor documentation to ensure compliance with state regulations and organizational standards.
• Participate in quality improvement initiatives and help maintain a safe, healthy environment for the individuals served.
• Respond to medical concerns and provide clinical guidance to residential teams as needed.
Qualifications
• Current Pennsylvania Licensed Practical Nurse (LPN) license in good standing.
• Previous experience in intellectual/developmental disabilities (IDD), behavioral health, community health, long-term care, or residential services preferred.
• Strong assessment, communication, and organizational skills.
• Ability to work independently while collaborating with interdisciplinary teams.
• Valid Pennsylvania driver's license and reliable transportation to travel between residential locations.
