Position Overview
Our client, a leading nonprofit human services organization, is seeking a compassionate and experienced Registered Nurse (RN) to support individuals with intellectual and developmental disabilities (IDD) living in community residential homes.
This is a field-based nursing role that provides clinical oversight, healthcare coordination, and nursing support across multiple residential locations. The RN partners closely with residential staff, physicians, specialists, and interdisciplinary teams to ensure individuals receive high-quality, person-centered care while maintaining compliance with applicable regulations and organizational standards.
Key Responsibilities
• Provide nursing assessments and ongoing health monitoring for individuals receiving residential services.
• Coordinate medical care with physicians, specialists, hospitals, pharmacies, and other healthcare providers.
• Review and monitor medication administration, physician orders, and treatment plans.
• Provide clinical guidance and education to Direct Support Professionals (DSPs) and residential staff.
• Assist with medical appointments, hospital discharges, follow-up care, and coordination of specialty services.
• Monitor individuals with complex medical and behavioral health needs.
• Ensure accurate clinical documentation and compliance with state regulations and agency policies.
• Participate in interdisciplinary team meetings and person-centered care planning.
• Respond to nursing questions and provide clinical consultation to residential teams.
• Assist with quality improvement initiatives and support regulatory readiness.
• Participate in an on-call rotation as needed.
Qualifications
Required
• Active Pennsylvania Registered Nurse (RN) license.
• Minimum of two years of nursing experience.
• Strong assessment, clinical judgment, and communication skills.
• Valid driver's license and reliable transportation.
• Ability to travel between residential homes throughout Berks County.
Preferred
• Experience working with individuals with intellectual and developmental disabilities (IDD).
• Experience in community health, behavioral health, home health, hospice, long-term care, or residential services.
• Familiarity with medication administration and healthcare coordination in community-based settings.
