Role Description The Patient Care Coordinator is a full-time, on-site role based in Reading, PA, responsible for supporting patients, caregivers, and clinical staff to ensure smooth delivery of home care services. Day-to-day tasks include managing appointment scheduling, confirming visits, and coordinating care plans between patients, families, and care teams. The coordinator will handle incoming and outgoing phone calls with professional phone etiquette, respond to patient inquiries, and provide clear information about services and care instructions. This role also involves updating patient records, verifying insurance or service eligibility as needed, and using basic medical terminology to communicate accurately with clinicians. The Patient Care Coordinator collaborates closely with nurses and caregivers to track patient needs, follow up on care concerns, and promote a positive, respectful experience for every patient and family.
Qualifications
• Candidates should possess strong Appointment Scheduling skills and the ability to manage calendars and visit confirmations efficiently.
• Candidates should possess excellent Phone Etiquette and customer service skills for handling patient and family communications in a professional, compassionate manner.
• Candidates should possess Care Coordination experience, including collaborating with caregivers and clinical staff to organize and monitor patient services.
• Candidates should possess familiarity with Medical Terminology to accurately relay information between patients, caregivers, and healthcare professionals.
• Candidates should possess a solid understanding of Patient Care principles, including respect for patient autonomy, safety, and comfort.
• Strong organizational and time management skills, with attention to detail in documentation and follow-up.
• Proficient computer skills, including use of electronic health records or scheduling systems.
• Excellent interpersonal and communication skills, with the ability to work in a diverse, team-oriented environment.
• Prior experience in home care, healthcare administration, or a related patient-facing role is preferred.
• High school diploma or equivalent required; additional training or coursework in healthcare or medical administration is a plus.
