Position Overview: We are currently seeking a compassionate, skilled, and self-motivated Registered Nurse (RN) to join our home health care team . In this full-time role, you will play a vital part in delivering high-quality, patient-centered care—helping individuals manage their health and recover safely in the comfort of their own homes. Key Responsibilities: Conduct comprehensive assessments and develop individualized care plans. Provide skilled nursing care, including wound care, medication management, IV therapy, and chronic disease education. Monitor patient progress and collaborate with the care team to ensure optimal outcomes. Educate patients and caregivers on health maintenance and safety practices. Accurately document all care provided in compliance with agency, state, and federal regulations. Willing to do on-call rotations, holidays, weekend, or weeknight. Qualifications: Active Registered Nurse (RN) license in Pennsylvania . Minimum of one year of clinical nursing experience; home health experience preferred but not required. Strong clinical and critical thinking skills. Excellent communication and documentation skills. Reliable transportation and valid driver’s license. Current CPR certification.
