In this vital role, you will deliver personalized care and support to clients in their homes, helping them with daily activities, health monitoring, and companionship. Your dedication will enable clients to enjoy a safe, comfortable, and independent lifestyle while receiving the highest quality care. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and record vital signs such as blood pressure to ensure patient well-being Provide medication reminders Prepare nutritious meals tailored to clients’ dietary needs and preferences, ensuring proper food safety practices Support patients with mobility using equipment such as Hoyer lifts or assisting with transfers safely Observe and report changes in health status or behavior Maintain a clean and safe environment by performing light housekeeping tasks like cleaning, laundry, and organizing Experience Prior experience working in home & community care or assisted living environments is highly preferred Experience caring for individuals with dementia, or memory care needs is a plus Familiarity with care plans, medication reminder, and vital signs documentation is beneficial Demonstrated ability to work compassionately with seniors or individuals requiring long-term care support Join our team to make a meaningful difference in the lives of those we serve!
