Candidates should be able to meet the following qualifications. 3-5 years of case management experience in a hospital-based setting 1 year of supervisory/management experience LCSW or LMHC required MSW - Masters Degree C-SWCM or C-ASWCM *Must be able to demonstrate expertise in hospital operations, discharge planning, care coordination and interdisciplinary collaboration. Hours: Full-time, 40 hours per week (M-F) Pay: $49.40/hr – $74.10/hr (based on years of experience) Key Responsibilities: Lead, coach, and develop a high-performing case management team while fostering a collaborative, patient-centered culture. Oversee daily department operations, including staffing, scheduling, payroll, productivity, and budget management. Ensure high-quality case management services by monitoring performance metrics, implementing quality improvement initiatives, and promoting best practices. Partner with physicians and interdisciplinary teams to optimize patient care, care coordination, and patient satisfaction. Mentor and support staff through onboarding, coaching, performance management, and professional development. Develop and implement departmental policies, procedures, and operational improvements to enhance efficiency and compliance. Monitor departmental financial performance, productivity, and resource utilization to meet organizational goals. Promote effective communication, teamwork, and exceptional customer service while leading staff through change and organizational growth.
