Job Summary We are seeking a passionate and energetic Early Intervention Community Specialist (Occupational Therapist, Physical Therapist, Speech-Language Pathologist, or Special Instructor) to join our dynamic team. In this role, you will deliver vital therapeutic services directly in the community, helping young children and their families navigate developmental challenges. Your expertise will empower children to reach their full potential through personalized care plans, family support, and collaborative case management. Duties Provide early intervention therapy services in home or community settings for infants and young children with developmental delays or disabilities. Assist families with activities of daily living (ADLs) to promote independence and functional skills. Develop, implement, and monitor individualized care plans tailored to each child's unique needs. Collaborate with multidisciplinary teams including physicians, educators, and other therapists to ensure comprehensive care. Support discharge planning by preparing families for transitions to outpatient or school-based services. Work with children who have autism spectrum disorder (ASD), Geriatrics, Pediatrics, or other developmental conditions to promote skill acquisition. Engage in case management activities to coordinate services and ensure timely delivery of therapy interventions. Skills Strong knowledge of physiology, anatomy, and medical terminology relevant to pediatric and adult populations. Experience working with individuals with disabilities across various settings such as inpatient units, outpatient clinics, skilled nursing facilities, home health, or school environments. Proficiency in assisting with ADLs and creating effective care plans for diverse populations including early intervention and post-acute care clients. Familiarity with discharge planning processes and case management strategies. Ability to work effectively with children with autism and other developmental delays in both clinical and community settings. Excellent communication skills for collaborating with families, caregivers, educators, and healthcare teams.
