Community Outreach Specialist Full-Time | Reports to: Vice President of Operations Base Salary: ~$50,000 + Aggressive Bonus Structure Area/Territory: Reading, PA (Berks County) Position Overview The Community Outreach Specialist is a key driver of growth, visibility, and community connection for Aging With Comfort home care agency. This role focuses on building a strong referral network, engaging directly with adults who may benefit from home care services, and creating meaningful partnerships with organizations, professionals, and community leaders. This is a relationship-centered, high-impact position for someone who thrives in the community, enjoys connecting with people, and is motivated by both mission and measurable results. --- Key Responsibilities: Community Engagement & Outreach Attend and host events within residential buildings where potential clients live, including tenant gatherings, community rooms, and onsite resource events. Participate in community-based activities such as food drives, health fairs, wellness events, and resource distributions to identify and engage adults who currently receive or may need home care services. Maintain a consistent, trusted presence in the community to increase awareness of home care services and support individuals through the enrollment process. Referral Network Development Build and maintain strong relationships with key referral sources, including: Supports Coordinators, Case Managers, Physician Offices, Rehabilitation Centers, Hospitals, Pharmacists Serve as a reliable partner for professionals seeking timely support for individuals in need of home care services. Partnership Building with Community Stakeholders Identify and collaborate with individuals and organizations actively engaged in the community, such as: State Representatives, Senators, County Assistance Offices, Medical Alert Companies, DME (Durable Medical Equipment) Providers, Local nonprofits and resource organizations Coordinate joint events, educational sessions, and outreach initiatives to expand access to home care services. Client Identification & Enrollment Support Proactively identify adults who may benefit from home care services through community interactions, partner referrals, and event participation. Work closely with internal teams to support the enrollment process and ensure individuals receive timely guidance. Track outreach activities, referral outcomes, and partnership engagement with accuracy and consistency. Ideal Candidate Outgoing, personable, and comfortable engaging with diverse community members. Strong relationship-builder with excellent communication skills. Motivated by mission and results, with the ability to work independently. Experience in outreach, community engagement, healthcare, or social services is a plus but not required. Organized, reliable, and passionate about helping adults access the care they need.
