As a vital member of our home health services, you'll coordinate patient assessments, develop personalized care plans, and ensure seamless communication with families and healthcare providers. Responsibilities Conduct comprehensive patient assessments including vital signs, physical examinations, and detailed health histories to determine individual care needs. Develop and implement personalized care plans aligned with physician orders and patient goals, ensuring adherence to all regulatory standards. Administer medications, injections, IV infusions, and perform wound care procedures such as suturing and ostomy management with precision and aseptic technique. Monitor patients’ conditions closely, managing acute pain, respiratory support (ventilators), and other complex medical needs within the home setting. Educate patients and their families on disease management, medication administration, nutrition, mobility assistance, and safety protocols to promote independence. Coordinate with interdisciplinary teams for discharge planning, case management, and referrals for specialized services such as hospice & palliative medicine or behavioral health support. Requirements Valid RN license in good standing with current state certification. Proven clinical experience in diverse settings such as hospital medicine, ICU, emergency room, or medical-surgical units. Critical care experience including ICU or trauma center background is highly preferred. Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/ICD-10), and diagnostic evaluation techniques. Experience working with individuals with disabilities or developmental delays is a plus; familiarity with long-term care environments like nursing homes or assisted living is beneficial. Excellent patient assessment skills combined with the ability to perform complex procedures including catheterization, tube feeding management, sterile processing techniques, and infection control practices. Ability to work independently in home settings while maintaining high standards of safety and professionalism; valid driver’s license required for travel between patient locations.
