Position Summary A Long Term Care Ombudsman serves as an advocate for residents of long-term care facilities, ensuring their rights are protected and concerns are addressed. This role involves investigating complaints, educating residents and families about their rights, and working with facility staff to resolve issues and improve care quality. Key Responsibilities · - Advocate for the rights and well-being of residents in long-term care facilities. · - Investigate and resolve complaints made by or on behalf of residents. · - Educate residents, families, and facility staff about resident rights and relevant regulations. · - Conduct regular visits to long-term care facilities to monitor conditions and speak with residents. · - Collaborate with facility administrators and staff to address systemic issues and improve care standards. · - Maintain accurate records of complaints, investigations, and resolutions. · - Provide training and support to volunteer ombudsmen. · - Participate in community outreach and education efforts. Qualifications · - Bachelor’s degree in Social Work, Human Services, or a related field preferred. · - Experience working with elderly populations or in long-term care settings. · - Strong communication, mediation, and problem-solving skills. · - Ability to maintain confidentiality and demonstrate empathy and professionalism. · - Knowledge of long-term care regulations and resident rights. · - Proficiency in Spanish is very helpful. · - Valid Pennsylvania driver’s license and reliable transportation. Work Environment This position involves regular travel to long-term care facilities and may require occasional evening or weekend hours. Work is performed both in office settings and in the field, with frequent interaction with residents, families, and facility staff. Required Clearances and Trainings Prior to start date, all Safe Environment Clearances must be received and reviewed, including FBI fingerprint, Pennsylvania State Police and Pennsylvania Department of Health and Human Services clearances. All clearances must be renewed in accordance with Diocese of Allentown and/or Commonwealth of Pennsylvania requirements for duration of employment. Prior to start date, completion of Mandated Reporter Training (MRT) and every 5 years thereafter or when required by agency, Diocese of Allentown, and/or Commonwealth of Pennsylvania. Prior to start date, completion of Protecting God’s Children Training (PGC). Completion of Commonwealth of PA's Ombudsman training program. Any additional requirements implemented by the Diocese of Allentown, the Commonwealth of Pennsylvania or specific grantors, for the duration of employment.
