Hands From The Heart is an established Medicaid certified homecare company and we are currently seeking an experienced Bi-lingual (Spanish) Homecare Case Manager to supervise their assigned Client's and their Caregivers in the Reading, PA area. The ideal candidate must be organized, possess strong interpersonal skills, and be willing to visit clients in their homes to ensure proper care is being delivered by the assigned caregivers in a fast paced environment. Duties include: Managing Client schedules. Overseeing the work of caregivers assigned to their Clients. Locate coverage for and assigning caregivers to open shifts and call outs. Supervising Caregiver performance. Work interdepartmentally with other staff to ensure that the client's needs are met. Provide up-to-date reporting to Senior Management. Must be available for on-call (rotated with other Case Managers). Requirements: Prior experience with HHA Exchange required. A driver's license and reliable transportation is necessary . Working knowledge of Microsoft Word, Excel, and Outlook. Possess excellent customer service and communication skills. Ability to work independently and in a group setting.
