We are currently seeking a compassionate and skilled Home Health Physical Therapist Assistant – PTA to provide personalized, one-on-one therapy services in a home setting throughout Lynchburg and the surrounding coverage areas. This position is ideal for a PTA who enjoys working independently while helping patients regain strength, mobility, balance, and overall functionality in their daily environment.
As a Home Health Physical Therapist Assistant – PTA, you will work under the supervision of a Physical Therapist to carry out individualized treatment plans for patients recovering from illness, injury, surgery, or chronic conditions. This role requires travel to patient homes throughout the service area.
Responsibilities of Home Health Physical Therapist Assistant – PTA:

Provide home-based physical therapy services under the direction of a supervising Physical Therapist
•
Assist patients with therapeutic exercises, stretching, balance training, gait training, and mobility activities
•
Monitor patient progress and report changes in condition to the supervising Physical Therapist
•
Reinforce and carry out individualized treatment plans established by the Physical Therapist
•
Educate patients and family members on exercises, mobility techniques, safety precautions, and home exercise programs
•
Assist patients in improving strength, endurance, coordination, and functional mobility
•
Document patient visits, progress, and treatment responses accurately and timely
•
Collaborate with interdisciplinary team members including RNs, LPNs, CNAs, OTs, and SLPs to support patient goals and continuity of care
•
Maintain compliance with home health regulations and company policies
Qualifications of Home Health Physical Therapist Assistant – PTA:
•
Graduate of an accredited Physical Therapist Assistant (PTA) program
•
Active PTA license in the state of Virginia
•
Minimum of 1 year of PTA experience preferred (home health experience preferred)
•
CPR certification and TB baseline screening required
•
Strong communication and interpersonal skills
•
Good clinical judgment and ability to work independently
•
Reliable transportation and willingness to travel throughout the service area
