Position Summary: Under general supervision of Medical Director, plans, develop, and implement therapeutic nutrition programs for children and adolescents. Provide group and community nutrition education programs.
Role Qualifications
Education:
• Master’s degree in food and nutrition from an accredited program or licensed as a Registered Dietician.
Experience
• Two or more years in public health or office nutritional counseling strongly preferred.
Skills
• Strong knowledge of dietary counseling regarding chronic disease factors and obesity required.
• Strong teamwork and community activism skills.
Location:
• Attendance is required during regular business hours at assigned CHDC site(s).
Other
• Bilingual a plus.
• Ability to work with diverse medically underserved population required.
Position Responsibilities
Clinical
• Provides nutrition assessments, prepares nutrition care plans, and educates and motivates patients to accept and follow programs without regard to their insurance status or ability to pay.
• Attend weekly health center staff clinical team meetings and other staff meetings as required. Participates in development of group practice standards, quality assurance objectives and activities, and presents didactic teaching material to other health center clinical team members.
• Develop culturally appropriate educational materials on nutritional issues for use with children, parents/guardians, and community health education programs. Evaluates and recommends nutrition education used by other health center staff.
• Documents all patient care, including face-to-face and telephone encounters and consults, appropriately in the medical record per health center protocols.
• Seek appropriate consultation and clinical supervision as needed from colleagues, including transfer of patients exceeding professional skills, abilities, or comfort.
• Works with social service agencies and other community groups to assess community nutritional problems and needs; develop and implement related community nutrition programs.
• Provides complete, accurate billing data for all encounters in a timely manner per health center policy.
• Maintains all health center required credentials at all times and provides health center with updated copies. Maintains and expands current level of professional knowledge of nutrition theory and practice through continuing education.
• Works with senior management team to develop practices leading toward an upward productivity trend.
