**REQUIREMENTS**
- Current state license as a Registered Nurse
- CPR certification
- 1-2 years experience as a Registered Nurse in a clinical care setting
- 2+ years Home health experience
- Homecare Homebase Software Experience
- OASIS Experience
- Proof of reliable transportation and insurance
**ROLES & RESPONSIBILITIES**
- Assess patients in various situations to identify their physical, psychosocial, and environmental needs.
- Develop and implement a Plan of Care ensuring quality and continuity of care.
- Educate patients and families on care, medications, and self-care practices.
- Supervise and provide guidance to home health aides to ensure quality service delivery.
- Monitor compliance with guidelines from third-party payers.
- Provide effective communication as a liaison among patients, family members, and healthcare professionals.
