Role Description
This is a full-time, on-site role located in Reading, PA, for a Primary Care Provider. The Primary Care Provider will deliver comprehensive medical care to patients, including preventive, chronic, and acute care. Responsibilities include diagnosing and treating illnesses, developing and documenting care plans, and collaborating with a multidisciplinary team to ensure optimal patient outcomes. The provider will also participate in patient education, community outreach initiatives, and quality improvement programs.
Qualifications
• Strong skills in Medicine, clinical judgment, and care planning
• Excellent Communication and customer interaction skills
• Demonstrated experience in providing patient-centered care, including assessment for Home Care needs
• Ability to provide training and educate patients and their families about health and wellness
• Commitment to serving a diverse population with compassion and cultural sensitivity
• Current, unrestricted medical license in Pennsylvania (MD, DO, NP, or PA)
• Board certification in Primary Care or related field is preferred
