The Registered Nurse (RN) conducts supervisory visits to assess the quality of patient care, provide guidance to direct care staff, and ensure compliance with agency policies and procedures in both home and facility settings. The RN also plays a key role in initiating client services for private home care, including conducting initial assessments and developing individualized care plans. Key Responsibilities: Conduct routine and as-needed supervisory visits to evaluate the quality of care provided by CNAs, Home Health Aides, and LPNs in home and facility settings. Assess patient needs and ensure care plans are appropriate, up to date, and accurately reflect current clinical conditions. Provide clinical guidance, training, and constructive feedback to direct care staff to ensure best practices are followed. Accurately document visit outcomes, care plan updates, staff performance, and any required corrective actions. Collaborate with physicians, care coordinators, and family members to ensure consistent, person-centered care delivery. Monitor compliance with agency protocols, state regulations, and nursing standards of practice. Initiate and oversee client services, including completing initial nursing assessments and developing individualized care plans. Qualifications: Active Registered Nurse (RN) license in the state of practice. Minimum of 1–2 years of experience in home health, private duty nursing, or a related clinical setting. Strong clinical assessment, communication, and documentation skills. Ability to work independently, manage time effectively, and provide compassionate, client-centered care. Familiarity with care planning, supervisory visits, and state home care regulations is preferred. Reliable transportation and willingness to travel to client homes and facilities as needed.
