Job Overview We at Everyday Home Care are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality patient care in the home settings. As an LPN, you will play a crucial role in supporting patients' health and well-being through direct care and collaboration with healthcare professionals. We currently have cases in the Robesonia Pa, area. Shift varies as we have much availability. Willing to discuss schedule as we know flexibility is important to our staff. Preferd Shift: Thursday – Sunday, Night Shift Responsibilities Provide direct patient care, including administering medications and treatments as prescribed. Conduct patient assessments and monitor vital signs to ensure optimal health outcomes. Assist with daily living activities and personal care for patients, including those with developmental disabilities or memory care needs. Document patient information accurately and maintain compliance with HIPAA regulations. Collaborate with healthcare teams to develop and implement patient care plans. Educate patients and families on health management, medication administration, and any healthcare needs. Perform various procedures in the home as needed, as prescribed. Support patients experiencing acute or chronic conditions through effective communication and empathy. Experience Candidates should have experience or willing to learn in one or more of the following areas to be considered for this role: Long-term care or home care settings. (Preferred, not required) Proficiency in EMR systems. Experience with Tracheostomy care and Feeding Tube care, is a preferred, not required. Willing to train.
