TriSource is seeking a compassionate and skilled Licensed Practical Nurses (LPN) to work within a residential group home for IDD adults located within the Reading, Birdsboro, and Fleetwood PA areas! Schedules: Monday- Friday 10p-7a Saturday + Sunday 10p-7a Pay Rate: $32/hour Qualifications: PA LPN License CPR/First Aid Driver's License TB test + Physical exam within the past 12 months Criminal background check High School Diploma or GED Responsibilities Provide direct patient care, including administering medications and treatments as prescribed by physicians. Monitor patients' vital signs and report any changes or concerns to the healthcare team. Assist with diagnostic evaluations and participate in the development of patient care plans. Conduct triage assessments to prioritize patient needs effectively. Support patients with activities of daily living, ensuring comfort and dignity. Collaborate with interdisciplinary teams to deliver comprehensive care in settings such as acute care, hospice care, and senior care facilities. Maintain accurate patient records and documentation in compliance with healthcare regulations.
