Clinical Pharmacist : Berks Community Health Center (BCHC) is seeking a dedicated Clinical Pharmacist to collaborate with providers and nursing staff to deliver safe, effective, patient-centered medication therapy. The Clinical Pharmacist plays a key role in managing chronic conditions, identifying high-risk patients, providing medication counseling, and ensuring optimal pharmaceutical care across our FQHC sites. Key Responsibilities: Monitor and optimize medication therapy for chronic conditions such as hypertension and diabetes Collaborate with multidisciplinary teams to improve patient outcomes Provide medication reconciliation, counseling, and clinical interventions Review labs and patient data to prevent adverse drug events Participate in quality improvement and patient safety initiatives Dispense medications in accordance with Pennsylvania State Law Education & Requirements: Graduate of an accredited School of Pharmacy Active Pennsylvania pharmacist license Board certification or board-eligible status preferred Excellent communication skills and ability to work effectively in a team environment Retail and/or clinical pharmacy experience preferred The ideal candidate demonstrates strong clinical judgment, professionalism, and a commitment to equitable, high-quality care for all residents regardless of economic status.
