Job Overview We are seeking a compassionate and dedicated Direct Care Worker to provide essential support and care to individuals with diverse needs, including seniors, individuals with disabilities, developmental disabilities, and those requiring specialized memory or dementia care. Responsibilities Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support. Provide medication reminders and assist with medication administration following prescribed care plans. Support residents with specialized care needs such as Alzheimer’s, dementia, autism, or other developmental disabilities through personalized behavior management strategies. Monitor vital signs and document patient information using Electronic Medical Record (EMR) systems to ensure accurate health tracking. Facilitate meal preparation and feeding while adhering to dietary restrictions and nutritional guidelines. Maintain a safe environment by performing light cleaning duties and ensuring proper infection control practices are followed. Operate equipment such as Hoyer lifts or other assistive devices to safely transfer residents requiring heavy lifting assistance. Support residents’ emotional well-being through social interaction, conflict management, and implementing individualized care plans. Drive residents to appointments or activities when necessary, ensuring safe transportation practices. Collaborate with healthcare professionals and case managers to coordinate comprehensive care services aligned with each resident’s needs. Requirements Prior experience working in assisted living facilities, nursing homes, home care environments, or with individuals with disabilities is highly desirable. Knowledge of medical terminology, HIPAA regulations, and patient confidentiality standards. Certification in First Aid and CPR is required; additional certifications in caregiving or nursing are a plus. Ability to perform light housekeeping tasks such as cleaning and maintaining a sanitary environment. Strong interpersonal skills with the ability to communicate effectively with residents, families, and team members. Physical ability to perform tasks involving heavy lifting and prolonged periods of standing or walking. Valid driver’s license preferred for transportation duties; clean driving record is advantageous. Flexibility to work various shifts including evenings or weekends as needed.
