Role Description
This is a full-time on-site role for a Nurse Practitioner located in Greenwood, SC. The Nurse Practitioner will provide high-quality primary care services to residents in senior living and skilled nursing facilities within the region. Responsibilities include performing medical evaluations, developing treatment plans, managing chronic and acute conditions, and collaborating with multidisciplinary care teams. The Nurse Practitioner will focus on enhancing the health outcomes and quality of life for older adults through compassionate, patient-centered care.
Qualifications
• Proven clinical skills in patient assessments, diagnosis, treatment planning, and chronic disease management
• Effective communication and interpersonal skills to build strong relationships with patients, families, and interdisciplinary teams
• Knowledge of value-based care models and experience in geriatric or long-term care settings preferred
• Ability to work independently and manage a caseload while ensuring high standards of care
• Active Nurse Practitioner licensure and certification in the state of South Carolina
• Proficiency in electronic medical record (EMR) systems and data documentation
• Compassion, empathy, and a commitment to improving the well-being of older adults
• Master’s degree in Nursing with specialization as a Nurse Practitioner (FNP, AGNP, or equivalent)
