CLINICAL CARE COORDINATOR
Position: Placement and Psychiatric Clinical Care Coordinator
Location: Reading, PA

Pay: $57,000
Position Summary
The Placement & Clinical Coordination Manager oversees foster care intakes and placements, ensures case file compliance, and coordinates psychiatric services. This role integrates clinical coordination, regulatory compliance, and program operations to support timely, appropriate, and child-centered foster care services.
Essential Duties & Responsibilities:
Referral Management & Intake Coordination
• Receive, review, and manage foster care referrals from county, state, and private agencies.
• Assess referral information for completeness, risk factors, clinical needs, and placement suitability.
• Coordinate intake decisions with program leadership and clinical staff.
• Ensure timely acceptance, denial, or redirection of referrals in compliance with agency policies and regulations.
Foster Home Matching & Placement Support
• Match children with foster homes considering:
• Clinical, emotional, behavioral, medical, cultural, and educational needs
• Foster parent skills, training, capacity, and preferences
• Collaborate with foster care staff to support placement stability and continuity of care.
• Participate in placement staffing and case consultation meetings as needed.
• Track placement outcomes and identify trends affecting placement stability.
File Review & Regulatory Compliance
• Conduct routine and targeted case file reviews to ensure compliance with:
• State foster care regulations
• Licensing and accreditation standards
• Agency policies and procedures
• Monitor documentation for:
• Medical and psychiatric services
• Treatment plans and assessments
• Court orders and consents
• Identify compliance gaps and collaborate with supervisors to implement corrective actions.
• Prepare documentation and reports for internal audits, licensing inspections, and quality assurance reviews.
Psychiatric Services Coordination
• Coordinate psychiatric appointments with the agency’s staff psychiatrist.
• Ensure all required consents, authorizations, and documentation are obtained and maintained.
• Track appointment attendance, follow-up needs, and medication monitoring requirements.
• Communicate relevant clinical information among case workers, supervisors, foster parents, and the psychiatrist while maintaining confidentiality.
• Support continuity of psychiatric care during placement changes or transitions.
Collaboration & Program Support
• Work closely with case workers, supervisors, foster parents, and clinical staff to support holistic service delivery.
• Participate in multidisciplinary team meetings as appropriate.
• Provide guidance to staff on compliance expectations for clinical and placement documentation.
• Assist with development and updates of policies related to intake, placement, and clinical coordination.
Qualifications:
Education
• Bachelor’s degree in social work, Psychology, Human Services, or a related field required
• Master’s degree preferred
Experience
• Minimum of 3–5 years’ experience in foster care, child welfare, or a related human services field
• Experience with placement decision-making, compliance monitoring, or clinical coordination strongly preferred
