About the Population Health Patient Engagement Coordinator Position
T he Population Health Patient Engagement Coordinator will work with the Population Health Manager to ensure the Population health management model of care addresses individuals' health needs at all points along the continuum of care through participation, engagement, and targeted interventions for a defined population.
The Population Health Patient Engagement Coordinator will participate in Community Health Centers' delivery of the highest quality Population Health and Accountable Care. In this capacity, they use culturally and linguistically appropriate methods to assist patients in completing preventative and supportive services identified by gaps in care to improve clinical health outcomes, quality performance, and incentive measures based on practice goals and payer contracts.

Duties Include But Are Not Limited To
• The Population Health Patient Engagement Coordinator will work under the direction of the Population Health Manager, review, assist with patient quality improvement outcomes through a data-driven approach, and apply critical thinking skills in developing patient engagement opportunities.
• Support patient engagement initiatives as assigned, facilitate outreach, and identify efforts to alleviate any barriers to care related to scheduling for patients and families' overdue preventative care services identified through payer/health center data.
• Identify non-compliant patients with gaps in care or lost to health center follow-up but attributed to health center panel, including contacting patients by phone, text, and other means, and documenting all patients encountered as appropriate.
• The Population Health Patient Engagement Coordinator must be able to turn patient data developed and pulled, from a defined list, into opportunities for outreach and engagement while educating staff members on patient populations identified and population health outreach initiatives.
• Work with all health center service lines and population health teams to achieve coordinated, patient-centered care.
• Use tools like population health applications such as Electronic Medical Recording, Practice Management systems, patient portal administration, HealthTalk A.I. platform, etc.
• Create outreach cohorts to address the closure of patient care gaps and improve quality performance across the health center and value-based care.
• The Health Patient Engagement Coordinator will work to improve rates of annual well-visit completion and overdue preventive screenings.
• Proactivelymanage payer panel roster management,patients non-compliant, overdue, or coming due for preventive screenings, missed care appointments, and lapses in the engagement of patient’s attributes.
• The Population Health Patients Engagement Coordinator will manage initial outreach activities for new patients recently added to insurance panels.
• Identify trends to barriers mentioned from outreach to patients and work with all pertinent health center staff to mitigate risk factors and encourage future visit attendance.
• Work with health center scheduling staff on availability and wait-listing opportunities while maintaining a thorough understanding of payer Medicaid and Medicare appointment access and accessibility standards.
• Monitor ongoing progress, generate population health reports, document support efforts, identify resources, and engage care delivery teams.
• Manage access to insurance payer platforms for access to panel rosters, care gap lists, and quality performance scorecards.
• Continue analysis of outreach method, frequency, and message content to determine optimal patient/family engagement strategy.
• The Population Health Patient Engagement Coordinator must maintain a professional role model in practice transformation and, under leadership guidance, effectively.
• All other duties as assigned.
Requirements
Qualifications, Education, and Knowledge
• Bachelor’s degree in a related health care field or equivalent work experience required
• One (1) year of experience in customer service or health care related field preferred.
• Proficient with Microsoft Word and Excel, EMR, EPM certification (or willing to complete Certification within Two years in the Population Health Department role)
• Comfort and flexibility working in a fast-paced health environment.
• Remote and attendance required at assigned CHDC site(s)

Our History
Community Health and Dental Care was formed through the generous funding of the Pottstown Area Health and Wellness Foundation along with various local businesses. Our doors opened in October 2008 with 39 medical and dental patients seen that first week and we’ve grown to see thousands of medical and dental patients each year. As a non-profit community health center, we have been organized to become the patient’s medical home for preventive care and to promote overall well-being, thus reducing the number of emergency room visits.
