Integrated Case Management
Job Summary
Responsible for providing support to Case Managers to ensure case management services, communications, and appropriate documentation is consistently processed, while serving as a liaison for patients, family of patients, and hospital staff to ensure patient satisfaction, experience, and quality of services are delivered. Responsible for ensuring compliance with regulatory requirements including identification and timely notification of Medicare patient rights, payer correspondence including coverage notices and denial/appeal rights. Requires the ability to provide an in-depth explanation of Medicare rules and regulations as it relates to patient status, discharge processes, and post-acute services to Medicare beneficiaries. Assists with providing technical and clerical support, to the case managers to timely transition patients to post-acute services in support of efficient patient throughput and hospital capacity.
Required Qualifications
- Require a minimum of two (2) years customer service experience in a healthcare or related setting.
- Require a High School Diploma, GED Equivalent, or Certificate; Associate's degree in related field preferred.

- Prefer knowledge of computer systems.
- Prefer previous healthcare experience with discharge planning activities.
- Prefer demonstrated ability to accurately document in the medical records and various case management systems/software.
- Prefer effective oral and written communication skills.
- Prefer demonstrated ability in establishing and maintaining effective working relationships.
- Prefer demonstrated initiative and self-direction.
- Prefer demonstrated computer, keyboard and data input skills.
