Description
The Social Worker  provides individuals, families and/or groups with the psychosocial support needs to cope with chronic, acute or terminal illnesses.  Services include advising patients and family caregivers. This role will also help with managing and coordinating the social and psychosocial needs of high risk, and high costs members to appropriate resources, services and programs. This position will provide Social Determinate of Health (SDOH) and/or Health Related Social Needs (HRSN) to the patient population being managed. This position also assists the RN Care Coordinator with the transition of care functions (SDOH) and/or Health Related Social Needs (HRSN). In order to accomplish this, this position evaluates and plans for comprehensive coordination of care and health services to manage members across the continuum. This position will collaborate with the integrated team to improve quality, reduce costs and enhance the member experience.

Responsibilities
Essential Duties and Responsibilities•    Evaluates or assists other team members in evaluating the psycho-social needs of the members •    Assists in identifying and utilizing information and services from community resources•    Develop and maintain working relationships with community agencies•    Coordinates the resolution of member concerns•    Provides timely response to member psychosocial needs•    Demonstrate empathy and strong negotiation skills when assessing needs•    Implement departmental policy and procedures to ensure effective care coordination program implementation•    Conducts member assessments, identifies gaps and interventions and coordinates ongoing care with the integrated team•    Develop personalized care plan with members•    Maintains accurate and confidential documentation•    Maintains knowledge of current regulatory, state/federal regulations that impact population served•    Performs other duties as assigned 
Qualifications
Required Education: Bachelor of Social Work degree from an accredited institution
