The Psychiatry and Behavioral Health Care Navigator will be responsible for providing care and guidance to high-risk patients to promote better health outcomes. The position will work to eliminate barriers to care by guiding, educating, and navigating patients to receive necessary health care monitoring and resources, both within Centra and externally. The position will be charged with building professional relationships with the patient and their families, local and regional healthcare providers to ensure smooth transitions and timely access to care for patients. The Psychiatry and Behavioral Health Care Navigator is expected to have extensive knowledge of local resources on order to successfully link patients and families to the appropriate Behavioral Health resources and supports based on their specific needs and insurance status. Required Education: Bachelor's Degree from an accredited institution or HS Diploma and at least five (5+) years of providing care navigation with Psychiatry and Behavioral Health programs/services. Required Experience: At least one (1) year of experience and involvement in community-based care coordination, experience facilitating patient engagement, and leveraging community resources to help get better health outcomes. Preferred Experience: Previous experience navigating healthcare systems and/or Behavioral Health services.
