Description
LPNs practice under the Virginia State Board of Nursing and under the supervision of a professional registered nurse. The Centra Medical Group (CMG) licensed practical nurse (LPN) coordinates and provides care to patients in a medical practice under the tenets of Centra's Professional Practice Model. The LPN collaborates with providers and members of the interdisciplinary team to provide high-quality patient care. The LPN coordinates the various care processes that are necessary to achieve desired outcomes. The LPN works to promote and sustain a culture reflective of Centra's mission, vision, and values. The practice of the LPN reflects the American Nurses Association's (ANA) Code of Ethics for Nurses, the Nurses Practice Act of Virginia, and the ANA Principles of Nurse Staffing.

Responsibilities
Assists with physical exams and health historiesEstablishes a compassionate environment by providing emotional, psychological and spiritual support to patients and familiesProvides health promotion, counseling and education to patients and caregivers. Identifies patient care requirements by establishing a rapport with potential and actual patients and other persons in a position to understand care requirementsAdministers medications, wound care, and personal health interventions at the direction of a providerMaintains accurate, detailed patient records and charts using an electronic medical records (EMR) systemCollects patient information to share with and collaborate with providers and interdisciplinary team membersMaintains safe and clean work environment by complying with policies and procedures, rules, and regulationsAdheres to CMG Quality Metrics driven by Centers for Medicare & Medicaid Services (CMS)Maintains patient confidence and protects operations by keeping information confidential and following Health Insurance Portability and Accountability Act of (HIPAA) regulationsMay perform other duties as assigned or requested and job specification can be modified or updated at any time
Qualifications
Required Education: Graduate of an accredited Practical Nursing ProgramRequired Certifications and Licensures: Hold a current, active license as a practical nurse in Virginia or hold a current multistate licensure privilege as a practical nurse. Hold a current, active American Heart Association Basic Life Support (AHA BLS) course completion card.
