Description
The RN Patient Placement Coordinator is responsible for continuity of operations within the facilities by overseeing all patient admissions and transfer activity while monitoring capacity both currently and prospectively. Facilitates patient referrals by physicians from outside the local area. Responsible for facilitating continuous throughput of patients from all points of entry by utilizing effective verbal communication swift decision making and deployment of ancillary services. Serves as liaison between clinical departments. Works within resource constraints and individual patient needs to secure the best possible placement for all patients. Facilitates PACE-on call after hours, weekends/holidays to support community services. Participates in Remote Home monitoring through monitoring patients and on-call services.

Responsibilities
Utilizes the nursing process as a frame of reference for the placement of patients in the most appropriate bed
Demonstrates the necessary knowledge and skills to address the care needs appropriate to age, spiritual and cultural needs of the population served
Serves as the command center for the organization with all patient flow within the hospital including ED, direct admits, same day surgeries, and transfers
Collaborates with the Flow Coordinator/Nursing Supervisor to address flow concerns throughout the organization
Facilitates patient referrals from physicians outside the area to local physicians
Serves as a resource for interpretation of policies and procedures as related to patient placement including, but not limited to placements of isolation or special care patients
Provides bed availability information to Incident Command during disasters
Coordinates and communicates system and patient needs with areas represented in the inpatient flow
Collaborates with members of the health care team to facilitate best placement
Facilitates communication between outlying and local physicians and arranges patient transport if needed
Demonstrates ability to work collaboratively with other departments by maintaining and communicating global picture of hospital capacity at all times
Works closely with all departments and physicians to ensure consistent processes in patient placement
Communicates frequently with Administrative Supervisors on all campuses to keep them aware of patient flow and potential problem areas
Notifies Staffing Office reporting census fluctuations that may impact staffing needs
Acts as central contact for decisions related to bed management
Provides On-Call services for PACE and Remote Home Monitoring to help support services in the organization and the community

Qualifications
Required Qualifications:
Required education: Associates Degree or higher from accredited School of Nursing
Hold a current, active license as a registered nurse in Virginia or hold a current multistate licensure privilege as a registered nurse.
Required Experience: Demonstrated leadership, supervisory, interpersonal and problem-solving skills. Must possess positive attitude and outstanding verbal communication skills
Required Certifications and Licensures: Licensed or eligible for licensure in the state of Virginia by the State Board of Nursing
Preferred Qualifications:
RN with minimum of two (2) years of clinical nursing experience with one (1) year in Critical Care
Bachelors of Science Degree in Nursing (BSN)
