Physical Therapist- Home Health
Provides medically directed physical therapy treatment to clients 18+ with injuries, illnesses, developmental issues, or social challenges, aiming to enhance independence and quality of life.
• Conducts assessments of clients' physical abilities and develops personalized treatment plans in collaboration with families, physicians, and community resources.
• Maintains all required competencies and certifications, including PA Physical Therapy License, BLS, and a valid driver’s license.
• Utilizes strong communication, interpersonal, and computer skills to deliver compassionate, patient-centered care.
• Demonstrates attention to detail, multitasking, organizational, and problem-solving abilities.
• Works as an effective team player in a supportive environment that values diversity and growth.
Education: Doctorate or relevant education & experience.
Experience: Relevant physical therapy experience.
Skills: Collaborative, customer service, medical terminology, Microsoft Office, and excellent interpersonal skills.
