Job Description

The Department of Health (DOH), Bureau of Community Health Systems is seeking a dedicated Community Health Nurse for our Southeast District. DESCRIPTION OF WORK As a Community Health Nurse, you will assess, plan, and implement public health services by providing educational and clinical services to help people improve their health. You will analyze county assessments to identify public health needs, develop public health initiatives, and ensure educational and clinical activities are data driven. You will also provide outreach and case management services to individuals and targeted populations to promote patient health and safety. This role requires effective communication to participate on district and local health committees as well as a genuine desire to advocate and support the development of public health initiatives. In addition, you will provide clinical services for immunizations, conduct case investigations of reportable diseases and outbreaks, and coordinate with community health organizations to i mplement disease prevention and control measures that control and prevent the spread of disease.

Work Schedule and Additional Information:
• Full-time employment, 37.5 hours per week
• Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
• Telework: You will not have the option to telework in this position.
• Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY QUALIFICATIONS
Special Requirements:
• You must possess an active authorization to practice as a Registered Nurse in Pennsylvania.
• If you possess an active temporary practice permit, you must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing.
Other Requirements:

• You must be able to perform essential job functions.
Legal Requirements:
• You must pass a background investigation.
• This position falls under the provisions of the Child Protective Services Law.
• Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
