The Prevention and Diversion Department is seeking compassionate and dedicated nursing professionals for the Children and Parents Together Always (CAPTA) Nurse Care Coordinator role. When a baby enters the world exposed to substances, and a mother is fighting to hold her life—and her child—together, they need more than services. They need someone to support them. That someone could be you.
About CAPTA: The CAPTA Home Visitation Program provides wraparound services for one of our most vulnerable populations: substance-exposed newborns (SEN) and young children affected by parental substance use.
As a Nurse Care Coordinator, you’ll stand alongside families, helping to:
• Reduce the number of substance-exposed newborns
• Preventing unnecessary removals from the home
• Promote healing, resilience, and long-term family well-being

Purpose of the Role: At CAPTA, we walk with families during one of the hardest and most beautiful times in their lives. As a Nurse Care Coordinator, you are the bridge between crisis and care bringing health guidance, emotional support, education, and dignity into the homes of families working to stay together.
What You'll Do
• Provide prenatal education and emotional support
• Guide mothers through childbirth preparation.
• Offer lactation education and breastfeeding support.
• Support postpartum recovery and adjustment.
• Infant care education and specialized care for substance-affected infants
• Encouragement for bonding and responsive caregiving
• Coordinate care with OB/GYNs, pediatricians, and specialists.
• Craft a comprehensive and customized Plan of Safe Care (POSC)

Hourly Pay Range : $30.00-$34.00
Requirements
We’re Looking For:
• Licensed Registered Nurse (RN) or Licensed Practical Nurse (LPN)
• A passion for working with families affected by substance use
• Completion of a Nursing certificate or degree from an accredited institution
• 2+ years of experience in a medical, hospital, or social service setting
List Of Other Minimum Requirements
Possess a valid Florida Driver’s License in good standing and be insurable by the agency’s current insurance carrier. Must meet state criminal background check requirements. Ability to work a flexible schedule. Knowledge of community resources.
