Job Summary
Responsible for providing support to Case Managers to ensure case management services, communications, and appropriate documentation is consistently processed, while serving as a liaison for patients, family of patients, and hospital staff to ensure patient satisfaction, experience, and quality of services are delivered.
Required Qualifications
• Require a minimum of two (2) years customer service experience in a healthcare or related setting.
• Require a High School Diploma, GED Equivalent, or Certificate; Associate's degree in related field preferred.
Preferred Qualifications
• Prefer basic knowledge of computer systems.
• Prefer previous healthcare experience with discharge planning activities.
• Prefer demonstrated ability to accurately document in the medical records.
• Prefer effective oral and written communication skills.
• Prefer demonstrated ability in establishing and maintaining effective working relationships.
• Prefer demonstrated initiative and self-direction.
• Prefer demonstrated computer, keyboard and data input skills.
• Prefer demonstrated knowledge of Windows software preferred.
