Job Description: The individual hired for this position provides in-person and/or telehealth outpatient mental health and rehabilitative services to children, families, and/or adults. They will provide person-centered, strength-based, trauma-informed, and recovery-oriented services in a caring and compassionate manner supporting individuals through a holistic approach to their healthcare. Responsibilities: Provides outpatient mental health and rehabilitative services including assessment, screening, diagnosis, treatment planning, case management, crisis intervention, safety planning, and ongoing treatment. Supports safe, quality-based behavioral health care services. Responds to a range of clients’ needs using differential diagnoses, appropriate treatment modalities, and a variety of intervention methods conforming with evidence-based best practices. Responds to emergent client needs, including risk and other safety assessments and referral for further evaluation. Engages clients from a person-centered, strength-based, trauma-focused, recovery-oriented holistic perspective, addressing social determinants of health. Prioritize development of plan of care with individuals in association with all community providers, family, and collateral supports identified by and for the individuals. Responds to a range of clients’ needs using differential diagnoses, appropriate treatment modalities, and a variety of intervention methods. Completes referrals to community resources and clinical services. Completes timely documentation. Maintains clinical records and effectively uses the established electronic health record in accordance with established clinic policies and procedures and applicable regulatory requirements. Participates, engages, listens, and learns in individual and team meetings.Evaluates individual position and program operation and communicates identified needs to supervisor. Reviews and adheres to all agency policies and procedures. Fulfills responsibilities as a NYS Mandated Reporter. Maintains compliance with New York State Office of Mental Health regulations and guidelines. Maintains and develops current professional skill-base appropriate to position. Completes all program and agency training requirements. Provide community-based training as needed. Represents the agency in the community and maintains relationships with community partners with courtesy and professionalism. Supports a culture that prioritizes professionalism, kindness, teamwork, and staff development. Contributes to the development of new programs and initiatives, and the pursuit of available funding for new or existing programming. Requirements: Applicants are expected to possess a current NYS licensure as an RN, LMSW, LMHC, LCSW, or LMFT with at least one year of experience in a behavioral health care setting. A licensed Master’s Degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or a related mental health discipline is preferred. New graduates in need of an additional limited permit placement for licensure will be considered. The ideal candidate will demonstrate strong verbal and written communication skills, problem-solving skills, and an understanding of systems with a global perspective. This individual also will possess strong clinical skills and an ability to work independently as well as in a teams-based approach with others.
