Communicates with physician's offices, referral sources, patients, and their families. Maintains a well-organized accessible patient record and EMR, which includes orders management, re-certifications, QI requirements and record analysis. Mails, tracks and files physician orders and certifications. Maintains regulatory compliance by disseminating medical information to collaborate with attending physicians, Skilled Nursing and Assisted Living Facilities with whom a treatment relationship exists. Releases Information as directed by the Release of Information Coordinator, and in accordance with Hospice & Palliative Care Charlotte Region policy. Assists staff with EMR needs. Understands office machines, front desk protocols, security camera, and alarm panel. Accepts incoming phone calls and disseminates messages accordingly. Receives incoming mail and disseminates to staff accordingly. Maintains forms file and duplicates as needed. In Regional Offices, ensures IDG manual is ready for weekly or bi-weekly meetings. Liaise with program assistants in regional offices, assisting as needed. Enters data into the Electronic Health Record (e.g., patient numbers, discharge data, transcribed data). Participates in audit and performance improvement activities by monitoring and reporting to appropriate staff and the Quality Improvement committee. Provides coverage for the VIA receptionist (Client Service Representative) as needed. Customer Service Has knowledge of and implements the principles and process for providing customer and personal services to outside customers and to VIA staff defined as customers as well. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction. Assumes responsibility for related duties as required or assigned. Ensures that VIA’s professional reputation is maintained and projected through job performance. Assumes responsibility for professional development and staying abreast of current trends in the field. Incorporates new information and methods into practice. Maintains current professional licensure or certification (if applicable). May perform other duties as assigned. Minimum Qualifications High School diploma required. Associate degree in health related field with Registered Health Information Technician (RHIT), or certification eligible preferred. Minimum of 1 year of related experience in medical records or equivalent exposure in health care facility/setting. Proficiency in Microsoft Office is required.
