Job Description
The Social Services Director is responsible for overseeing the social services program, ensuring compliance with regulatory standards, and developing effective discharge plans to address the social, emotional, and practical needs of residents. This role also involves working closely with residents, families, and interdisciplinary team members to promote well-being and enhance the quality of life for our residents.
Requirements
Responsibilities:
• Supervise and manage the social services department, including staff training and performance evaluations.
• Conduct assessments to identify residents' social and emotional needs and develop appropriate care plans.
• Collaborate with the interdisciplinary team to ensure comprehensive care for residents.
• Lead discharge planning efforts by coordinating community resources and services.
• Provide education and support to residents and families regarding transitional care and available resources.
• Ensure compliance with federal, state, and local regulations pertaining to social services in healthcare.
Qualifications
• Must have a Bachelor's degree in a human-services related major, including sociology, gerontology, psychology, rehabilitation counseling or special education; AND
• Must have One year in a healthcare setting where the individual is working as a supervised social worker who works directly with individuals.
• Strong knowledge of person-centered care principles and community resources.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively in a fast-paced environment.
