Nurse- Case Manager (RN)
HealthCare Support is actively seeking a Care Manager- RN to fill an opening with 2 Skilled Nursing Facilities in Berks County, PA .
Daily Responsibilities for RN Case Manager:
• Assist the provider/team with benefit determination associated with Medicare part A, part B, and part D benefits
• Responsible for collaborating with the primary advanced practice clinician, (APC) and facility to identify and respond to patient change in conditio
• Mon – Fri 8am – 5pm
Required Qualifications for RN Case Manager:
• Graduate of an accredited School of Nursing program
• Active and Unencumbered Registered Nursing License in PA
• 1+ years of recent clinical experience of Geriatric or Skilled Nursing case management
Preferred Qualifications for PRN Case Manager:
• Bilingual in English/Spanish
• Meeting the medical needs of patients with complex behavioral, social and/or functional needs

Pay Details: $52 – $57 an hour
