Physical Therapist PRN
• Provides nursing care to residents in a long-term care, nursing home, or rehab center setting.
• Reports to Director or Assistant Director of Nursing; no direct reports.
• Implements resident care plans, administers medications, monitors health, and documents care.
• Communicates changes in resident condition to appropriate medical staff.
• Orients and supports unlicensed personnel as assigned; assists with admissions, discharges, and transfers.
• Follows safety, infection control, and compliance policies; maintains confidentiality and promotes residents' rights.
• Must be a licensed LPN/RN with CPR certification and 1 year of nursing experience preferred.
• Requires flexibility, good communication, teamwork, and ability to manage difficult situations.
• Physical requirements include moving, lifting, and responding in emergencies.
