Position: Health Services Supervisor/Licensed Practical Nurse (LPN) Immediate Supervisor: Community Supports Manager (CSM) and Registered Nurse (RN) Qualifications: The person must be an LPN currently licensed and capable of carrying out the nursing service activities deemed appropriate for the persons served. The individual should have knowledge and experience in the field of comprehensive homecare, day services, and 245D non-residential services. Must also be able to meet the following qualifications: a. Valid driver’s license and meet qualifiers to be insured by EON, Inc.’s vehicle insurance provider. b. Must not be disqualified by appropriate criminal background studies. c. Complete all orientation and training needs as written in policy. d. Meet the physical requirements as outlined in this position description. e. Possess good oral and written communication skills and organizational skills. f. Able to read and write in English. g. Basic knowledge of Microsoft Office and Internet Browsers Preferred Qualifications: Ability to acquire familiarity and understanding of all applicable Minnesota Department of Health (MDH) and/or Department of Human Services (DHS) rules and regulations, and company policies governing Assisted Living, Adult Day Services, Structured Day Services, and non-residential services. Possess a participatory leadership style. A self-reliant individual who can accept direction and work in multiple services with minimal supervision. General Responsibilities: To provide health services to person served under the supervision of the Registered Nurse and Community Supports Manger. The position is responsible for maintaining the quality and adequacy of nursing practice; for coordinating nursing and training programs, for recognizing and assisting in solving professional, administrative, and supervisory problems in an assigned area. Work involves responsibility for supervising the provision of quality nursing care given to service sites. To assist the person served directly with ADL's, medication administration, and medical appointments. To document person served health concerns, attend in-services, staff meetings, consultations as directed, to collaborate with other agency programs, and person served family members. Hours Worked: Schedule is to be based on meeting the needs of the programs assigned and will require direct care. The Health Services Supervisor may request a change in hours. The Community Supports Manager or Operations Director may approve and/or mandate a change in hours based on program and person served needs. All persons in this position are required to work weekends and holidays if needed. Participate in “ON-Call” responsibilities as designated by Community Supports Manager.
